Prior to using the support request form, we ask that you please review and utilize any relevant resources within this website in order to provide effective additional supports.
Please use the contact request form below to receive additional support in connecting your needs to the appropriate district/ community-based support.
Forms are confidential and are forwarded by email to the leadership team in the Counseling department. Only those individuals with a need to know will be informed of the form contents. A District Counselor will be in contact with you/the student by the end of the next school day.
Please remember you can always speak with your teacher or principal to discuss your concerns.
